Ryther provides several different wraparound support programs to families who may need assistance in coordinating both mental and physical healthcare for their child. These programs are for youth who need a high level of care and their families.
The Health Home program is for children and young adults (ages 0-21) who have Apple Health insurance and who have multiple health conditions and may have complex care needs. The goal of Health Home is to improve the quality and coordination of care across systems, increase confidence and self-management of health, decrease the risk of hospitalization, and to provide a single point of contact to bridge systems of care.
WISe is a service for individuals up to 21 years of age with complex behavioral health needs that would benefit from a higher level of care or is at risk of out of home placement. This program uses a team approach to provide intensive mental health services to support youth and their family to meet their goals. Team members include natural supports (such as family, friends, and religious leaders) and the professionals who work with the family (such as counselors, schools, CPS, and probation officers). This program requires a time commitment from the youth and family of at least 10.5 hours per month.
